Complaints and Investigations

Key Takeaways:

  • The College observed an increase in complaints with respect to RNs employed by agencies and practicing in the area of aesthetics.
  • Delays in employer reporting, as noted in previous years, have trended downward in 2025.
  • The Inquiry Panel heard several cases in 2025. Cases coming before the Inquiry Panel were related to matters involving competence, criminal charges, unbecoming conduct, and unethical behaviour.
  • This year, the College has seen another significant increase in the number of complaints received by the CEO/Registrar, along with a notable increase  in urgent investigations being referred to the Complaints Investigation Committee.
  • There has been a shift in trends typically investigated by Professional Conduct. An increase in matters related to registrant competency concerns led the forefront of our investigations in 2025.

In 2025, we received 215 complaints, employer reports or information about RN practice and conduct, which is the highest number of complaints on record for a single year in the College’s history. All 215 matters were triaged within the legislated time frame to determine the level of risk to the public and the course of action to address the alleged conduct. There are currently 235 matters under investigation because 20 matters from before 2025 are still in progress.

Abeyance means a state of temporary disuse or suspension.

Practice Settings of the 165 Referred Complaints

The most highly reported allegations in 2025 include:

  1. Inappropriate Action (52 or 31.5%) – Doing something you ought not to. The action or conduct is not the expected action of a reasonable and prudent nurse (e.g. not upholding aspects of the nursing process, not adhering to employer policies).
  2. Communication (25 or 15.1%)– RNs are accountable and responsible to effectively communicate with the client, other health care providers, and the public, integrating the nursing perspective into professional communication.
  3. Medication Errors (23 or 13.9%) – A medication error is any preventable event that may cause or lead to inappropriate medication administration resulting in possible patient harm by a health care professional.
  4. Fitness to Practice (19 or 11.5%) – The registrant is suffering from a physical or mental condition or disorder, including an addiction that impairs their ability to engage in the practice of registered nursing in a safe and effective manner.
